Reimbursement Request Form
Use the LPE PTA Reimbursement Request form to request reimbursement from the PTA for teacher mini grants, club supplies, program materials, etc.

To submit a reimbursement request:
- Complete the PTA reimbursement request form (download and print one or grab one from the front office).
- Include the original receipt. For online purchases, include a printout. All receipts should be itemized and include the date of purchase, the total, and the form of payment.
- Submit the form and receipt(s) to the PTA mailbox. Allow 4 weeks for reimbursement.
